When moving department-owned computers and printers to a new location,
please fill out the form below. Please fill out and submit a separate
form for each piece of equipment.
If you have received
departmental permission to take a department-owned computer home,
please fill out the form below and download NCSU Equipment Tracking
Home Use Authorization Form CA-2, as per NCSU
regulations. The department fills out the top half of the form,
keeps the original and gives the person taking the computer home a
copy. A copy of the form has to go to the CAMS Coordinator (Rosina).
When the equipment is returned, the department completes the bottom
half of the original form, keeps it on file
and informs the CAMS Coordinator that it has been returned.

Please note: If you are planning
on taking an office computer home, you might want to touch base with
one of our IT staff first. When setting up departmental computers, we
configure them for use in the College's network environment. For you
to use the computer at home, it may be necessary to create a local account
for you on the computer, grant you admin rights, change the network
settings, etc -- things we can't do after you've taken the computer
home.
