Student Profile Sheet
The student profile sheet is used by the department for dissemination of information to students.
Patent Agreement
All faculty, staff and students engaged in University related or sponsored research shall sign a Patent Agreement document. A signed patent agreement must be on file in the Graduate School in order for the Plan of Work to be reviewed and approved. The department will make a copy and forward the original to the Graduate School.
Master's Plan of Graduate Work
A Master's Plan of Graduate Work is developed by the advisory committee with the student. The course work to be taken by the student and the thesis topic, where applicable, must be approved by the student's committee and the Director of Graduate Programs in the student's department or program. This should be done prior to completion of one-half of the credits on the plan.
Doctoral Advisory Committee Plan of Graduate Work
The Doctoral Advisory Committee Plan of Graduate Work must be approved by the Director of Graduate Programs of the major department and the Graduate School. In addition to the course work to be undertaken, the subject of the student's dissertation must appear on the plan; and any subsequent changes in committee or subject or in the overall plan must be submitted for approval.
Doctoral Committee Modification Form
The doctoral committee modification form is used to make changes to the committee originally established. The form must be turned in to the department for approval by the Director of Graduate Programs. It is then forwarded to the Graduate School for approval.
Revision to Plan of Work
The Revision to Plan of Work form used to make any changes to the original Plan of Work. Only CHANGES are to be noted using the added and deleted columns on the form. If the semester/yr. Is the only change, it would be noted in both the added and deleted columns (i.e. EAC 703 03 hrs. Spring 01 in the deleted column and EAC 703 03 hrs. Fall 02 in the added column) The form must be signed by all committee members prior to submission, unless the student is changing only the semester and/or year a course was taken.
Request for Approval to Schedule Doctoral Oral Examination*
NOTE: Preliminary and Final are the same form, check the correct box on the form.
Preliminary Comprehensive Examination: Upon satisfactory completion of the written portion of the preliminary examinations and after completion of all course work relevant to the examination, authorization for the preliminary oral examination is requested from the Graduate School. This examination is conducted by the student's advisory committee and is open to all graduate faculty members. A doctoral student is admitted to candidacy upon passing the preliminary examinations without conditions or after fulfilling any conditions specified by the advisory committee.
Final Oral Examination: The final oral examination is scheduled after the dissertation is complete except for such revisions as may be necessary as a result of the examination, but not earlier than one semester or its equivalent after admission to candidacy and not before all required course work has been completed or is currently in progress. The examination consists of the candidate's defense of the methodology used and the conclusions reached in the research, as reported in the dissertation. It is conducted by an examining committee, which consists of the student's advisory committee. This examination is open to the University community.
* This form has to be in the Graduate School two weeks prior to the date of the scheduled exam and the department requires three weeks in advance to allow time for the Director's signature.
Request for Option B Checkout
The Request for Option B Checkout form for Master of Education students os used by the Graduate School as a check-out form prior to graduating. An approved Plan of Work and signed Patent Agreement form must be submitted prior to the Option B. See academic calendar for submission deadline. Director of Graduate Programs submits requests for graduation Request for Option B Checkout to the Graduate Dean no later than 30 working days after the first day of the semester in which the student is taking the last course on his/her Plan of Graduate Work and anticipates graduation.
Diploma Order Request Card
The Diploma Order Request Card form used by all students planning on graduating in the given semester. See academic calendar for submission deadlines for master’s students. Doctoral students will submit form with their exam paperwork on day of final oral exam in the semester they graduate.
